IPS is a rapidly growing healthcare organization providing mental health services to hospitals, rehabilitation centers, and long-term acute care facilities, in addition to a robust outpatient practice.
We are seeking an Acute Care Coordinator to oversee and support a team of providers in the hospital setting. This role ensures provider productivity, monitors documentation and communication with referring providers, and proactively identifies and resolves any gaps in care coordination. The ideal candidate must have a strong understanding of hospital operations, medical practice workflows, and experience managing healthcare providers.
Key Responsibilities:
Provider Oversight & Coordination
- Manage a team of hospital-based providers, ensuring they have a productive clinical day.
- Track and manage incoming referrals to ensure timely and appropriate follow-up.
- Facilitate seamless communication between providers, referring physicians, consulting groups, and hospital administrators.
- Monitor provider documentation, ensuring accuracy, completeness, and compliance with hospital and practice standards.
- Identify and address any missed opportunities in patient care coordination.
Patient Intake & Access
- Monitor and direct a high-volume mental health access line for individuals seeking mental health support.
- Screen and schedule patients appropriately based on their specific needs in a professional and courteous manner.
- Provide patients with support and guidance as needed.
Administrative & Operational Support
- Enter data into EHR systems in a timely and accurate manner.
- Monitor multiple systems simultaneously to ensure efficient workflow.
- Maintain and update all Acute Care Dashboards and Trackers daily.
- Develop and implement processes to improve provider efficiency and communication
- Professionalism & Teamwork
- Maintain professional behavior and uphold high standards of customer service.
- Build and maintain positive working relationships with all IPS staff.
- Assume additional tasks, duties, and responsibilities as assigned by the supervisor or designated IPS staff.
- High school diploma required; minimum of two years’ experience as a unit clerk or patient service coordinator in a hospital setting.
- Strong knowledge of hospital operations and medical practice workflows.
- Experience managing healthcare providers in a hospital or similar setting.
- Ability to effectively triage provider calls to ensure appropriate assignment.
- Excellent customer service, communication, and organizational skills.
- Ability to manage multiple tasks simultaneously in a fast-paced environment.
- Empathetic personality with attention to patients’ needs and concerns.
- Strong attention to detail and ability to track multiple moving parts.
Typical working demands:
This position is located in Augusta Georgia, in office. Will be 3 10-hour days (Friday - Sunday) and 2 5 hour days (weds/thursday) to start.
